Four Seasons Nursing and rehab Centers admissions
office is open Monday through Friday from 8:00am
to 5:00pm. If you would like to make a referral
or have any questions regarding the admissions process,
our friendly and knowledgeable staff can be contacted
at 718-927-6300 ext 320. If your loved one is in
a hospital, a PRI can be faxed directly to the admissions
office at 718-927-0136. Four seasons is also accepting
on line referrals through ECIN. We at Four Seasons
Nursing & Rehabilitation Center would also be
more than happy to give you and your family a tour
of our facility, evening hours are available. |
